Support Coordination is a service that helps you understand and use your NDIS plan.

At I CAN SEE YOU, we work with you and the people around you, to build confidence, connect you with the right supports, and help you work toward your goals.

It’s not just about organising services. It’s about feeling supported, listened to, and empowered to make choices that reflect your values and your life.

Support Coordination is a collaborative process. That means working together with you, your family, and your care team to make sure your supports are clear, consistent, and aligned with your goals.

Julie is based in Greater Geelong and has strong connections with a wide range of local services and professionals, making it easier to find quality supports that truly suit your situation.

Checking your eligibility

Who is eligible?

To access Support Coordination, it must be included in the Capacity Building funding section of your NDIS plan.

If you’re not sure whether it’s included, have a chat with your NDIS planner or Local Area Coordinator. If it is part of your plan and you’re ready to get started, you can make a referral here.
