To access support form I Can See you, a current NDIS plan s required with funding allocated for a support coordinator. If you have an NDIS plan but no funding then please contact me directly to discuss your individual circumstances to request this in your next plan review.

Support coordination is listed in your plan under the section of Capacity Building Supports. The funding for support coordination, cannot be taken form any other line item in your plan and so this must be specifically listed on your plan. If you are unsure if you have this listed in your plan, l am happy to discuss this with you and explain if you do or do not have this funded

A support coordinator can help you with the following, but this is not an exhausted list and each person will require different tasks to be completed by the support coordinator.
Here is an over view of what a Support Coordinator can do for you:
Support you to understand and implement the funded supports in your plan
Help you to 'optimise' your plan ensuring that you are getting the most out of your funded supports
Help build your ability to exercise choice and control, coordinate supports and access your local community
Help link you to community, mainstream and other government services
Focus on supporting you to build your skills and independence (the fancy word to describe this is 'capacity building')
Find and connect you with NDIS providers
Assist you in negotiating with providers about what they will offer you and how much it will cost out of your plan
Ensure service agreements and service bookings are completed
Assist you in planning ahead to prepare for your plan review- write implementation, progress , support coordination reports as well as change in circumstances, end of plan and a RORD. Here is an over view of what a Support Coordinator CAN do for you:

Yes, you can, as long as you have the consent from the participant or the plan nominee. You are also welcome to call and discuss your individual needs without sharing your plan or signing a service agreement.
